Opening a Centennial Credit Account

Instructions
Follow these steps to set up your Centennial account:
- Download the credit application (right mouse click: "Save Linked File As" or "Save Linked File to Desktop")
- Type requested information directly into the fillable PDF or print form and complete by hand (please print).
- Email your completed application to accounting@centennialglass.com or by fax to 613-736-6564
