Follow these steps to set up your Centennial account:

  1. Download the credit application (right mouse click: “Save Linked File As” or “Save Linked File to Desktop”)
  2. Type requested information directly into the fillable PDF or print form and complete by hand (please print).
  3. Email your completed application to accounting@centennialglass.com or by fax to 613-736-6564
